Behavioral Health Homes (BHH)
The Adult Behavioral Health Home (BHH) is a holistic service intended to help adults manage their behavioral health and physical health needs. Services are provided by a team of individuals comprised of Health Home Coordinators, a clinical team leader, a nurse, and a peer. The team provides case management, symptom management, skills building, coordination with healthcare providers, and support from peers. The BHH services are available to individuals with MaineCare coverage. They are offered in an individual’s home, community, or outpatient locations in Lewiston, Bridgton, Oxford, Rumford, Farmington.
